Policies
Do you accept insurance ?
Yes, if you click on the insurance link on the left you will find
detailed information regarding insurance options. Please note
that your copay is due at the time of service. If you have a
deductible it will be due when your plan calls for it. In the
event that your insurer has not paid your open bill within 60 days of
invoice your are responsible for the bill.
What is your cancellation policy ?
In order to ensure that our time is used to its best efficiency we
ask that if you need to cancel an appointment that you do so at least
24 business hours prior to the scheduled appointment. If you
miss an two appointments a $20 fee is charged before the appointment
can be rescheduled.
What is your returned check fee ?
Our fee for return items is $27.
Do you accept credit cards ?
Yes, we take both debit and credit cards (with the exception of
Discover).
Do you offer a cash discount ?
Yes, when the cost of the services rendered exceed $100 a 5%
discount is offered if the bill is paid at the time of service.
What is your privacy policy ?
We comply with all government regulations regarding privacy.

this policy from the Forms page of this website.
Do you refer patients to specialists ?
In certain instances such as complex orthodontia, implants, or
extractions we find we can best serve our patients by referring them
to a specialist. We have working relationships with many
specialists and are happy to help you find the best one for your
specific situation.
What happens if I am late ?
If you are more than 15 minutes late for an appointment it may
have to be rescheduled.
My divorce decree says my former spouse is required to pay for all
medical services
For cases involving divorce, the parent that brings the child to
the appointment is responsible for payment. The office will not
bill the other parent.
