Appointments

Our office is open for patient appointments from 7:00AM to 5:00PM Monday, Tuesday & Wednesday and 7:00AM to 2:00PM on Thursdays.  You can make an appointment by calling the office at (770)834-6663.

If you are unable to call us during office hours to make your appointment you may also use the e-mail form below to request an appointment.  Please make sure you give us both your phone number and your e-mail address so we can contact you with the different options for times.
